School Registration

Registration will be by appointment only for enrolled families in August.

​

• Sign-up for your registration appointment using Sign-Up Genius. 

• Forms must be turned in at this time.​

• Please pay Student Service Fees at this time.

 

If have not enrolled but are interested in ICA for your child(ren), please call 708-562-5580 x 102 or email jczubernat@immanuel-hillside.org for more information or to apply.  

 

​The Registration packet can be previewed below along with school supply lists. 

Hard copies of forms were mailed to enrolled families on July 17, 2023.

State Requirements: 

1. Updated Physical - PK, Kindergarten, and 6th grade students

2. Updated Dental Visit - Entering grades K, 2nd & 6th

3. Updated Eye Examination - Kindergarten, required by the state

4. Concussion Form - All students

5. Sports Physical  - Students who wishes to participate in sports
