Registration will be by appointment only for enrolled families in August.

Hard copies of forms were mailed to enrolled families on July 17, 2023.

State Requirements:

Updated Physical - PK, Kindergarten, and 6th grade students

Updated Dental Visit - Entering grades K, 2nd & 6th

Updated Eye Examination - Kindergarten, required by the state

Concussion Form - All students

Sports Physical - Students who wishes to participate in sports
