School Registration

Registration packets will be mailed to enrolled families by July 18, 2025. 

​​

​​State Requirements: 

1. Updated Physical - PK, Kindergarten, and 6th grade students

2. Updated Dental Visit - Entering grades K, 2nd & 6th

3. Updated Eye Examination - Kindergarten, required by the state

4. Concussion Form - All students

5. Sports Physical  - Students who wish to participate in sports 

6. Certified Copy of Birth Certificate
